*Do I have behavioral health coverage?

*How does my deductible work with behavioral health services?

*How many sessions are covered in my plan per year?  Can the provider request more if needed?

*What is the coverage amount per therapy session?

*What is my co-pay?

*Do I need a referral by the Primary Care Physician?

*If I want to see a behavioral health care provider that is not covered under this insurance, can I pay for the bill, provide documentation to you and receive reimbursement for the service through my plan?  If so, will I be reimbursed in the full amount?
